Voters that are permanently physically incapacitated and unable to make it to their polling place, may qualify to automatically receive an application for an absentee ballot before every election for the next five years.
If you believe you qualify for this service, please visit our website at www.dekalbclerk.com to download the “Disabled Voter Application Form.” This form must be completed and returned with your physicians sworn statement indicating that you are eligible to receive this service or provide your disability card issued by the State of Illinois for verification.
Voters may also call the Elections office at 815.895.7147, or visit our office at 110 East Sycamore Street, Sycamore, Illinois, 60178 to sign up for the program.
