You can help lower your risk of breast cancer with recommended screenings, keeping a healthy weight, exercising and other lifestyle decisions. One in every eight women born today will be diagnosed with breast cancer at some time during her life.
Are you ready to “Take Control”? Please take a few minutes to read the educational material and complete the attached risk assessment survey. Based on the results of the survey, fifty women will be invited to receive eight free individualized lifestyle coaching sessions by professional staff from KishHealth System to help you reduce their risks for Breast Cancer.
The program is being provided by the DeKalb County Health Department in collaboration with KishHealth System. Funding for “Take Control” is made possible through a grant from the Illinois Department of Public Health, Carolyn Adams Ticket for the Cure Community Grant Program. All data collected will remain confidential and will not be made available to any other healthcare providers.
